Common Issues for Claim Denial

  • Incorrect code used - Always use 83861

  • CLIA Number not transmitted properly
  • Determine the appropriate modifier to be used for the second eye tested

  • The payer doesn't recognize the new code

  • Not in scope of practice or not contracted for payment

  • 3rd party payers may not have established payment policy for this code
