Common issues for claim denial
- Incorrect code used- Always use 83861
- Determine the appropriate modifier to be used for the second eye tested
- The payer doesn't recognize the new code
- Not in scope of practice or not contracted for payment
- 3rd party payers may not have established payment policy for this code
